MedSpeak Inc.'s Data Privacy Statement

By participating in the focus group discussion for digestive enzymes and answering the associated online form, I willingly state that I have read MedSpeak Inc.'s Data Privacy Statement and express my consent for MedSpeak to collect, record, organize, update or modify, retrieve, consult, use, consolidate, block, erase or destruct my personal data as part of the mentioned project. I hereby affirm my right to be informed, object to processing, access and rectify, suspend or withdraw my personal data, and be indemnified in case of damages pursuant to the provisions of the Republic Act No. 10173 of the Philippines, Data Privacy Act of 2012 and its corresponding Implementing Rules and Regulations. MedSpeak, Inc. recognizes its responsibilities under Republic Act No. 10173, also known as the Data Privacy Act of 2012, with respect to the data it collects, records, organizes, updates, uses, consolidates or destructs from participants in this project. The personal data obtained from this project are entered and stored within MedSpeak's authorized information and communications system and will only be accessed by authorized personnel and those of the sponsors of this project. MedSpeak has instituted appropriate organizational, technical, and physical security measures to ensure the protection of the respondents' personal data. Furthermore, the information collected and stored from this project shall only be used for the focus group discussion project mentioned. MedSpeak shall not disclose the respondents' personal information to anyone other than those specified in this document without the respondents' consent and shall retain this information over a period of not more than two years for the effective evaluation and review of the project, and for planning and organizing future activities.
